Fill out the Requisition Form. Please write legibly, fill out all required fields, sign and date, and have patient sign and date as well
Fill out the tube label with all the information required
On the non-dominant hand, select a puncture site on the middle or ring finger as shown on the marked areas on the image above
Wipe the puncture site using an alcohol pad and allow the finger to air dry
Remove the cap from the lancet by twisting once, firmly pulling the cap apart from the lancet casing
Firmly place the opening of the lancet on the puncture site. Without hesitation press the release button on the lancet to engage the needle and puncture the skin. (The needle will automatically retract)
Immediately after puncturing the site, firmly squeeze the tip of the finger to induce a drop of blood to pool on the surface of the finger. Hold the collection tube in a HORIZONTAL position and bring it to the drop of blood until the drop is suctioned into the tube. Avoid touching the surface of the skin
While maintaining the collection tube in a HORIZONTAL position AT ALL TIMES, squeeze the tip of the finger on and off while collecting each drop into the tube. Repeat process until the blood reaches the end of the tube (600 µl mark). If the puncture site stops bleeding, and specimen volume is insufficient, repeat steps 3-8 with a new set of supplies using a new puncture site. Be sure to ship all specimens collected
Once the tube is filled, you may set it down vertically on a flat surface. With a gauze/cotton pad, press firmly on the puncture site. Once bleeding stops, place a Band-Aid over the site
After finishing with the patient, slowly remove the inner section of the collection tube by unscrewing, and place the inner section on a gauze pad provided. Take the red cap provided and screw it on the collection tube
Place the tube label on the collection tube
Place the specimen in the biohazard bag. Collect all of the biohazard waste in the small Ziploc bag provided. Put the small Ziploc bag with the waste in the biohazard bag along with the specimen. Place the requisition form on the pouch located on the back of the biohazard bag labeled as “Place Paperwork Here.”
Place the packaged Biohazard Bag (containing the specimen and Biohazard waste) inside the cardboard box, and place the cardboard box into the pre-paid FedEx shipping bag
Contact FedEx for a pick-up or take your sample to the nearest FedEx office and drop it off at any of their indoor drop-off boxes. In order ensure sample integrity, it’s important that you absolutely DO NOT USE ANY OF THE OUTDOOR BOXES
